Nurses provide skilled nursing care to assist acutely ill clients in their home with a physician order. Personal care assistance in the home by Home Health Aide (CNA) and/or nurse.
Nurses provide health follow-up for adults with chronic, stable health problems in the home, clinic, or office.
LT-101 functional assessment for eligibility to home and community based services, and long term care provided on request.
Case management, skilled nursing services, and home health aide services provided based on eligibility.
Adult immunizations consist of flu, pneumonia, and tetanus. Immunizations are administered during clinics, on a drop-in basis, and during special clinics such as Health Fair.
